Federal regulations require that you meet all Satisfactory Academic Progress (SAP) standards to remain eligible for federal student aid. Based on your academic record, you are not successfully meeting one or more of these standards. Failure to meet both standards by the end of the current semester will result in your disqualification from further financial aid.
GPA: Maintain a cumulative GPA (Grade Point Average) of at least 2.00.
PROGRESS: Successfully complete at least 66% of all units attempted with a passing grade. Courses for which you received a grade of “F”, “NP” or “W” were not successfully completed.
In addition to the above, you are expected to complete an associate's degree or transfer program within 72 attempted units or a certificate program within 30 attempted units.
You should monitor your grades and academic records on MyGCC on an on-going basis to determine if you are meeting the Satisfactory Academic Progress standards required for financial aid. We support your efforts to be successful in school and recommend that you see your Academic Counselor regarding your Student Education Plan, career plans and options for improving your grades. Students receiving services from EOPS (Extended Opportunity Program & Services) or CSD (Center for Students with Disabilities) should meet with their Academic Counselor in that office. |

Exhausted |
Your appeal for additional classes was approved in a prior semester and you were placed on a Financial Aid contract. The contract course list mailed to you included all classes eligible for receipt of additional financial aid. Our records indicate that you have either received a grade or a "W" on all your approved courses and no classes remain available for payment or your contract has expired. Therefore, your eligibility for financial aid at Glendale Community College has been exhausted.

Please provide a SIGNED copy of your and your spouse's (if married) federal income tax return (including ALL schedules). Be sure to put your GCC I.D. number on the top of each return. |

Provide an IRS Tax Return Transcript and all W-2's for you (and your spouse, if married). Be sure to put your name and GCC I.D. number on the top of each transcript. If you do not have a copy of your W-2's, you should contact your employer(s) to request a duplicate.
To complete an online request for the tax transcript go to Order a Transcript. You will need to provide the taxpayers SSN, date of birth, street address and Zip code. The type of transcript you should request is the "Return Transcript".

Amended Tax Return |
Either you have indicated that you filed an amended tax return or we have found errors that require you to do so. If all information on your federal return was correct as originally filed, please submit a confirming letter from the IRS or an official letter from your tax preparer providing IRS code citations and a clear explanation of your compliance with IRS rules.
Students who have amended their federal income tax return must submit:
1. A copy of the original Form 1040 sent to the IRS and all W-2 forms
2. A copy of the corrected Form 1040 sent to the IRS
3. A copy of the Form 1040X sent to the IRS

| Verify Active Duty Status |
You indicated on your FAFSA that you are currently serving in the U.S. Armed Forces or are a National Guard or Reserves enlistee who is on active duty for other than state or training purposes.
You must provide one of the following documents to verify your Active Duty status:
1. Current military orders
2. Your military ID
3. A letter (on official letterhed) from your commanding officer
4. A copy of a current DD-214 (if it cleary indicates your active duty status) |

| Verify High School Graduation |
You have been selected by the Department of Education for verification (or re-verification) of the high school graduation requirement.
Bring the original of one of the following documents to the Financial Aid Office for photocopying:
1. A high school diploma
2. A final high school transcript showing your graduation date
3. A GED certificate or GED transcript
4. An academic transcript showing that you successfully completed a two-year program acceptable toward a BA degree
5. A secondary school completion credential for homeschooling if state law requires homeschooled students to obtain the credential
6. A transcript signed by your parent or guardian that lists the secondary school courses you completed, and clearly stating your successful completion of a secondary school education in a homeschool setting |

| Verify Selective Service |
A recent computer match between Selective Service and Federal records failed to confirm that you have registered with Selective Service. Therefore, you must provide written proof to our office that you have registered.
If you are a male that has not previously registered and are between the ages of 18 and 26, you can register on the web at www.sss.gov. If you have previously registered, you can also verify your registration at www.sss.gov. A printout from the online verification screen is acceptable documentation.
Students who entered the United States after their 26th birthday are exempt. Please bring in documentation (I-94 or Passport) which clearly shows your date of birth and the date that you entered the United States. If you did not register or cannot obtain written proof of your registration through the online verification web site, then you must contact Selective Service to resolve this matter before you can receive any financial aid. |
