DE Course Proposal: DE Addendum Form

Distance Education (DE) Addendum Form Approval Process:

A course shall have a Current COR and DE Addendum to be offered in 2021 and beyond.

From Title 5 § 55206. Separate Course Approval. "If any portion of the instruction in a new or existing course is to be provided through distance education, an addendum to the official course outline of record shall be required. . ."

General DE Addendum Form

The DE Addendum Process: 

  1. Division Curriculum & Instruction (C&I) Representative documents each Division Course Outline of Record (COR) that is approved by C&I.
  2. Division C&I Rep and/or the DEC contacts the Division Committee on Distance Education (CoDE) Rep with approved COR information and requests that the Distance Education (DE) Addendum Form for anytime DE scheduling, (or the DISTANCE EDUCATION (DE) ADDENDUM FORM: EMERGENCY DECLARATION ONLY for scheduling during an emergency only [only where appropriate]) be filled out and submitted for approval from the DE Addendum Subcommittee or CoDE.
  3. CoDE Representatives will contact instructors of record of the approved COR and assist the faculty member to fill out the Individual DE Addendum Form accurately.
  4. The DE Coordinator will receive the submitted DE Addendum Form, review it for accuracy, and forward it to the DE Addendum Subcommittee for review and approval (winter/summer) or for recommended approval from CoDE (spring/fall).
    1. NoteIt is recommended that the CoDE Representative review all DE Addendum Forms before they are submitted to the DE Addenda Subcommittee in order to increase consistency and accuracy and to decrease the number of DE Addenda that are returned to faculty for revision.

Why do we need the DE Addendum Form?

(a) If any portion of the instruction in a new or existing course is to be provided through distance education, the course outline of record or an addendum to the official course outline of record shall address how course outcomes will be achieved in a distance education mode and how the portion of instruction delivered via distance education meets the requirement for regular and substantive interaction as specified in section 55204.

 

(b) The course design and all course materials must be accessible to every student, including students with disabilities. The distance education course outline of record or addendum shall be approved according to the district's adopted curriculum approval procedures.
 
The addendum shall be separately reviewed and approved according to the district's adopted curriculum approval procedures.
