4 Food Stamps/SNAP Verif. You indicated on your FAFSA or CA Dream Act Application (for AB540 students) that a member of your household (or your parents' household, if dependent) received Food Stamps/SNAP.The household member who received Food Stamps/SNAP should complete the form.

Documentation can include a "Notice of Action" letter from Social Services indicating receipt of Food Stamps/SNAP by the household member (see the Food Stamps/SNAP Verification form description on the FAO website under Required Forms & Documents)OR completion of the Food Stamps/SNAP Verification form.

If an error was made on your financial aid application and no household member received Food Stamps/SNAP, you should print out the Food Stamps/SNAP Verification form and indicate on the form that an error was made.The form must be signed and dated by you (and a parent, if dependent).
5 Cal Grant

Cal Grant recipients who are not enrolled for Fall and/or Spring semesters must request a Leave of Absence.

Schools are no longer authorized to report a Leave of Absence for students with a NEW Cal Grant award. Consequently, students who have a NEW Cal Grant award and are not enrolled must request a Leave of Absence through WebGrants4Students on-line portal.

Students wishing to postpone payment of their Cal Grant funds until they transfer to a 4-year school may also request a Leave of Absence.If you will be transferring to a university, we recommend that you not receive Cal Grant disbursements in excess of two years at GCC.For assistance in determining the best options for your situation, you should contact CSAC at (888) 224-7268.

If you have requested a Cal Grant Leave of Absence (LOA) for one or more semesters for the current award year, this checklist is on your To Do List to ensure that no automatic disbursements of Cal Grant are made to you.

THIS CHECKLIST WILL NOT AFFECT DISBURSEMENT OF ANY OTHER FINANCIAL AID THAT YOU HAVE BEEN AWARDED OTHER THAN YOUR CAL GRANT.
6 Cert. of Household Size List all members of the household who will live with you from July 1 through June 30, and who will receive more than half of their support from you.

DEPENDENT STUDENTS: List people living in your parent's household who your parent's will be providing their support.

INDEPENDENT STUDENTS: List people living in your and your spouse's (if married) household who you will be providing their support.
7 Consortium Agreement You can request that your concurrent enrollment at another (secondary) college be taken into account when determining your financial aid eligibility for the current semester at GCC. Complete the Student’s section, have a GCC Academic Counselor complete the Academic Counselor’s section, and then take the form to the secondary college for completion of the secondary college’s section.Return the completed form to the Financial Aid Office with a copy of your current enrollment at the secondary college.

Note: A completed Consortium Agreement and current proof of enrollment at the secondary college must be received by the Financial Aid Office prior to the last day of the semester for which payment is being requested.
8 Contract Dismissal

You are not eligible for any financial aid because you failed to complete a contracted class for which you received financial aid.

If there are circumstances beyond your control which contributed to your failure to complete a contracted class, you may appeal this dismissal by submitting a Dismissal Appeal form outlining the circumstances causing your dismissal.All appeals will be reviewed, not all appeals will be granted.To regain eligibility an appeal is required.

This status does not affect BOG fee waiver eligibility or your financial aid status at other colleges.

10 Dependency Override Packet

Status)
If unusual circumstances exist which make it impossible for you to have contact with your parents, you may be eligible for financial aid as an independent student.The Financial Aid Office may be able to change your dependency status.Examples of unusual circumstances are:

1. A parent is incarcerated or institutionalized

2. You moved out of your parents' home due to adverse or abusive home conditions when you were under 18 years of age

3. For the majority of your childhood you were raised by people other than your parents

If your family situation involves an unusual circumstance such as those described above, you may request a review of your dependency status. Read the instructions carefully and submit all requested information and documentation.

All requests will be reviewed by the Financial Aid Office, but not all requests are granted.
11 Dependency Verif. Form The Financial Aid Office is attempting to submit a correction to your FAFSA applicationinformation on your behalf. Unfortunately, you left one or more of the dependency questions on your application blank.We are unable to continue processing corrections without completing these blank dependency questions.We need you to verify the accurate response to these questions before we can continue processing your financial aid.
12 Educational Goal Form

Your current Educational Goal in college records does not indicate that you are enrolled at Glendale College for the purpose of obtaining a degree or certificate offered by GCC or to transfer to a 4-year college or university.If this goal does not accurately reflect your educational objective, you should make an appointment to see your Academic Counselor to complete the Educational Goal Change form.
13 Enrolled in Grad Program

You indicated on your FAFSA or CA Dream Act Application (for AB540 students) that you will be enrolled in a master's or doctorate program. If this is incorrect, complete the appropriate section of the Status Change form.

For 2017-2018 if you, your parents (if dependent) or your spouse (if married) did not and were not required to file a 2015 tax return, complete the appropriate Statement of Non-Filing.

To request tax transcripts and/or proof of non-filing from the IRS for the tax year 2015,you cancomplete an online request at Get Transcript - Get Transcript ONLINE.Once the transcript or verification of non-filing is received, bring it to the Financial Aid Office.

If you are unable to complete an online request for a tax transcript and/or proof of non-filing you should complete the Request for Transcript of Tax Return (Form 4506-T) and mail it to the appropriate IRS office.Once the transcript or verification of non-filing is received, bring it to the Financial Aid Office.

18 No Fed Aid - FAFSA Item #23
(Statement of Eligibility)
You might not be able to receive federal student aid if you have been convicted of selling or possessing illegal drugs, if the drug offense for which you were convicted occurred while you were receiving federal student aid (grants, loans, and/or work study).

If you have been convicted in the past, this does not automatically mean that you are ineligible for federal student aid.The Student Aid Eligibility worksheet will help you determine your eligibility.For more information about Drug-Related Convictions, call the Federal Student Aid Information Center at 1-800-4-FED-AID (1-800-433-3243).
19 Cal Grant
Form G-35
At the time you completed your FAFSA, you (or your parents, if dependent) were not required to provide asset information.However, this information is required to finalize your Cal Grant eligibility.

The California Student Aid Commission (CSAC) sent you an Asset Verification Form (G-35) to be completed and returned to the Commission.The Financial Aid Office cannot disburse any Cal Grant funds to you until the Commission receives and reviews this form to make their final determination of eligibility.

If you have any questions, you should contact the Commission at (888) 224-7268.

20 Parent Stmt of Living Exp Have your parents complete the form by listing expenses that your parents incurred between January and December.If amounts differed from month to month, provide an average for the 12-month period.Do not include any expenses covered by food stamps or housing assistance.
21 Parents Marital Stat Verif The marital status you reported on your FAFSA or CA Dream Act Application (for AB540 students) for your parent(s) is not consistent with information reported on other forms/documents received.

Have your parent(s) complete the Parents' Marital Status Verification form as of the date you filed your financial aid application.
22 Parents' Untaxed Income Complete this form by providing information for untaxed income received by your parents.
23 Status Change Form This form may be used for the following:

- Requesting a Federal Work Study award
- Declining Federal Work Study awarded
- Cancelling ALL financial aid
- Informing the Financial Aid Office that you will not be enrolled in a Graduate Program (see form #13)
- Informing the Financial Aid Office of a correction in name or SSN

24 Prior Associate Degree

You indicated on your Admissions application that you have earned an Associate's degree.To regain eligibility an appeal is required. Complete the Term Dismissal appeal and make an appointment to see your Academic Counselor.Students who have successfully appealed are paid only for classes necessary to complete their educational goal.

If you have not earned an Associate's degree, you should correct this information in the Admissions Office. Once corrected, complete the Statement of Prior Associate's Degree form, then bring the completed form to the Financial Aid Office to meet with an Advisor. The Advisor will determine if any other documentation will be required to verify the information you are reporting.

This status does not affect BOG fee waiver eligibility.
25 Prior Bachelor Degree

BA Degree)
A review of your records indicate that you reported on your Admissions application or your financial aid application that you have a Bachelor's degree.As a result, you may be ineligible to receive Federal Pell Grant, Federal SEOG or Cal Grant.Complete the Statement of Prior Bachelor's Degree form, then bring the completed form to the Financial Aid Office to meet with an Advisor. The Advisor will determine if any other documentation will be required to verify the information you are reporting.

Student's who have a Bachelor's degree who wish to apply for federal work study or student loans must have an approved SAP Term Dismissal appeal and be on contract. Complete the Term Dismissal appeal form (#32) and make an appointment to meet with your Academic Counselor

This status does not affect BOG fee waiver eligibility.
26 Income Data Rev - DEP

In order to determine federal aid eligibility you must submit all required documents and submit verification from your parents of their refusal to provide information.

31 Student's Untaxed Income Complete the form by reporting any Untaxed Income received by you and your spouse (if married).Untaxed income includes payments to tax-deferred pension and savings plans; payments to IRA's; child support received; tax-exempt interest income; untaxed portions of IRA distributions; untaxed portions of pensions; housing, food and other living allowances; Veterans' noneducation benefits; untaxed income such as worker's compensation or disability benefits; any other money received by or paid on your or your spouse's behalf.
32 Term Dismissal

Students whose educational goal is to receive a BA or AA/AS degree or transfer who do not complete their educational goal within 72 units are placed on Financial Aid Term Dismissal.In addition, students whose educational goal is to earn a vocational certificate without transfer who do not complete their educational goal within 30 units are placed on Financial Aid Term Dismissal.

33 Selective Service Verif A recent computer match between Selective Service and Federal records failed to confirm that you have registered with Selective Service. Therefore, you must provide written proof to our office that you have registered.

If you are a male with a valid Social Security number that has not previously registered, and are between the ages of 18 and 26, you can register on the web at www.sss.gov.

AB540 students who do not have a valid Social Security number cannot use the Selective Service online system.However, you will be given the option to be registered with the U.S. Selective Service System when completing a CA Dream Act Application.

If you have previously registered, you can verify your registration at www.sss.gov.A printout from the online verification screen is acceptable documentation.If you do not have a valid Social Security number you will not be able to verify your Selective Service status online.

Students who entered the United States after their 26th birthday are exempt.Please bring in documentation (I-94 or Passport) which clearly shows your date of birth and the date that you entered the United States.

If you did not register or cannot obtain written proof of your registration through the online verification web site, then you must contact Selective Service to resolve this matter before you can receive any financial aid.

34 Untaxed Income Verif You or your parents (if dependent) should complete Section A of the form and then take it to the agency providing the benefits for completion of Section B.
35 Verif of Rental Property Based on documents submitted to our office, you or your family own rental property other than your primary residence.Complete a Verification of Rental Property form for each property listed on Schedule E of the federal tax return.Attach supporting documents for the current market value and outstanding mortgage debt.
36 Verification of Assets Additional information is being requested in order to determine your eligibility for student aid.You received a form identifying the issue(s) that need to be addressed.Complete the form with values as of the day you signed your original FAFSA or CA Dream Act Application (for AB540 students). This information being requested is due to one or more of the following:

- You or your parent's (if dependent) left the questions requesting asset information on your financial aid application blank.

- The amount of interest income listed on your or your parent's (if dependent) tax return conflicts with the amount of savings provided on your financial aid application.

- The amount of dividends listed on your or your parent(s) tax return conflicts with the net worth of investments provided on your financial aid application.
37 Verification of Support
(Verification of Support of Dependents)
According to federal regulations "dependents" are children or other persons who live with you and receive MORE THAN 50% of their living expenses and other financial support from you between July and June.We are unable to determine if the dependent(s) included on your FAFSA or CA Dream Act Application (for AB540 students) meet this definition. Complete the form by listing the qualified dependent included in the number in household on your financial aid application.A separate form must be completed for each dependent.

DEPENDENT STUDENTS: Have your parents complete the form for the person or persons that they support.

INDEPENDENT STUDENTS: Complete the form for the person or persons you (and your spouse, if married) support.
38 Verify Other Enrollment

You indicated other family member(s) in your household will enroll in College in at least 6 units.Complete the College Enrollment Verification Form for each family member (other than your parents, if dependent) that is enrolled in college and attach proof of registration.Proof of registration must include the students name, the name of the institution, the semester and the total units in which they enrolled. Online printouts are accepted if they include all of the above information.

In order to be eligible for federal and state financial aid at Glendale Community College you must be enrolled in a degree, certificate, or transfer program of study listed in the GCC catalog.Your educational goal in college records does not meet the requirements for an eligible program of study.

To be eligible to receive aid, you must have an approved appeal with the Financial Aid Office.All appeals will be reviewed but not all appeals may be granted.

42 Reg Stu at Other College

Federal regulations state that a student may only receive financial aid from the college/university from which they will transfer or receive a degree. You indicated that you will have transferred or are receiving your degree from a college other than Glendale Community College and are therefore not eligible to receive aid at GCC.

This status does not affect BOG fee waiver eligibility.
43 Not Enrolled
(Status Change Form)
Thank you for applying for financial aid. Our records indicate that you were not enrolled for the Fall and/or Spring semester.Consequently, we have cancelled your request for federal financial aid.

If you plan to attend Glendale Community College for the Summer session, you must complete a Status Change form so we can reactivate your file.
44 SAP Dismissal
(Dismissal Appeal Form)
Federal regulations require you to meet all Satisfactory Academic Progress (SAP) standards to remain eligible for federal student aid.Based on your academic record, you did not successfully meet one or more of the following standards and are no longer eligible for financial aid:

1. GPA: Maintain a cumulative GPA (Grade Point Average) of at least 2.00. You will not regain financial aid eligibility until your GPA is at least 2.00.

2. PROGRESS: Successfully complete at least 67% of all units attempted with a passing grade. Courses for which you received a grade of “F”, “NP” or “W” were not successfully completed.

3. ACADEMIC STANDING: Meet the Glendale Community College standards for Academic Good Standing (see "Scholastic Information and Regulations" in the college catalog). Students placed on Academic Dismissal by the Admissions Office are automatically disqualified from financial aid eligibility until they regain good academic standing with the College.

Your financial aid eligibility will be reinstated when you are meeting all of the above satisfactory academic standards.

This status does not affect BOG fee waiver eligibility.
45 Verify Child Support Paid You and you or your parents (if dependent) indicated on your FAFSA or CA Dream Act Application (for AB540 students) an amount for child support paid.You are required to provide information regarding who the child support was paid to, the names of the children for which child support was paid and verify the amount paid.

You or your parents (if dependent) must complete the Verification of Child Support Paid form.
46 Identity/Education Purpose You, the student, must appear in person at the Financial Aid Office at Glendale Community College (GCC) to verify your identity by presenting a valid government-issued ID, such as, but not limited to, a driver's license, other state-issued ID, or passport.

In addition, you must sign a "Statement of Educational Purpose" in the presence of a Financial Aid Office official at GCC.The statement will be provided when you come to the Financial Aid Office.

If you are unable to appear in person at the Financial Aid Office at GCC, you may print out Form #46 and take it to a Notary Public with a valid government-issued photo ID as described above.Be advised that Notary Publics often charge a fee for their services. Do not complete any part of the form until you are in the presence of the Notary Public.
47 Loss of CA College Promise Grant (Formerly known as BOG Fee Waiver) Appeal Form
State regulations mandate that students who are not maintaining satisfactory academic/progress standing (on academic and/or progress probation for two or more consecutive primary semesters) lose eligibility to use their CA College Promise Grant (CCPG)(Formerly BOG Fee Waiver) until satisfactory academic/progress standing is regained or a petition to have CCPG eligibility reinstated is approved.Our records show that you are not maintaining satisfactory academic/progress standing and therefore are not eligible to use your CCPG.

If you feel you have DOCUMENTED extenuating circumstances, you may petition to have your CCPG eligibility reinstated by using the “Loss of CCPG Appeal Form”.

APPEALS OF LOSS OF CCPG ELIGIBILITY WILL NOT BE APPROVED FOR A SEMESTER THAT HAS ALREADY ENDED.
48 Loss of Enrollment Priority and/or BOG Fee Waiver Appeal Form State regulations mandate that students who are not maintaining satisfactory academic/progress standing (on academic and/or progress probation for two or more consecutive primary semesters) lose eligibility to use their Board of Governors Enrollment Fee Waiver (BOG) until satisfactory academic/progress standing is regained or a petition to have BOG eligibility reinstated is approved.Our records show that you are not maintaining satisfactory academic/progress standing and therefore are not eligible to use your BOG.

If you feel you have DOCUMENTED extenuating circumstances, you may petition to have your BOG eligibility reinstated by using the “Loss of Enrollment Priority and/or BOG Fee Waiver Appeal Form”.

APPEALS OF LOSS OF BOG ELIGIBILITY WILL NOT BE APPROVED FOR A SEMESTER THAT HAS ALREADY ENDED.

CCCG Academic Counseling Cover & Assessment
As a Cal Grant eligible student at GCC, you may qualify for a Community College Completion Grant (CCCG), a financial aid award designed to encourage faster completion of your degree or certificate.

This form should be completed by the student and the counselor and submitted to the Financial Aid Office by the required deadline.
