Equipment pickup, drop-off and proficiency test hours:

- Monday through Thursday | 1:00 pm to 4:00 pm (by email appointment only)

Equipment pickup, returns and proficiency test important info: 

- All appointments, including returns, must be scheduled with the Support Technician by email at least 24 hours in advance

- All email correspondence with the Support Technician regarding checkout must be sent from your GCC student email

- If you fail a proficiency test you will need to wait 24 hours before you can retake the test

- All documents required for checkout must be emailed to the FTVM Support Technician at least 24 hours before pickup

- Physical copies of documents required for checkout will not be accepted

 

 


 

PREPARE TO BORROW EQUIPMENT:  Find out how to register to borrow equipment and take proficiency exams below.

Before you will be able to check out any equipment, you will need to set up an account with the FTVM Instructional Support Technician which will require providing by email:

1.     A signed copy of FTVM Equipment Checkout Agreement. Downloadable document:

2.     Front and back images of your GCC ID (the photo ID center is located on the 3rd floor of the Sierra Vista building)

3.     A scanned copy or image of a govt' issued photo ID (i.e. driver’s license, passport, state ID card, military ID, etc.)

4.     Proof of enrollment (this can be a screenshot or scanned copy of your course schedule for the current semester)

*All required documents for checkout must be sent to the Instructional Support Technician electronically via email at least 24 hours before your initial pickup. Physical copies of documents will not be accepted.

 

To Provide a digital signature you can:

• print, sign and take a photo

• sign digitally via pdf

• use a phone app to sign the form.  Either by uploading it or taking a picture. Some great FREE   

    apps are “Adobe Fill & Sign,” “DocuSign,” “SignNow,” and “SignEasy”

• use a website that you can allows you to upload and sign via the web:  https://www.pdffiller.com/

 

1.    Proficiency exams must be passed before you can receive any of the following equipment:

- Canon 90D

- Sony FX6

- Sony a7S III

- Zoom H4N

- Sennheiser Wireless Lavalier Mic

- Tascam 70D  

2.    Proficiency exams must be scheduled with the Support Technician by email at least 24 hours in advance.

3.    You must get 100% on the proficiency exam in order to pass.

4.    If you fail a proficiency exam you will need to wait 24 hours before you can retake the test.

5.    Proficiency exams are given by the Media Arts Instructional Support Technician or by a student worker.

EQUIPMENT CHECKOUT:  Checkout protocol, pickup/return schedules, late returns and equipment damage below. 

REQUESTING EQUIPMENT

All equipment requests must be made with at least 24 hours notice.

1.     Secure the proper equipment request form for the class. Understand that only certain equipment is allocated for each class. Keep in mind that the equipment you request may not be available due to a number of reasons.

2.     Ensure that you have passed a proficiency exam to prove that you are proficient with equipment you intend to check out.

3.     Turn in the fully completed Request Form with requested pick-up time to the FTVM Instructional Support Technician: mdoyle@glendale.edu.

4.     Wait to receive a confirmation with dates and pickup up times and other pertinent information from the Instructional Support Technician.

Checkout/in Time Periods:

Monday - Wednesday: Check out for equipment on Monday and it’s due back on Wednesday.  If you check out equipment on Wednesday, it is due back Monday.

Tuesday - Thursday Check out for equipment on Tuesday and it’s due back on Thursday.  If you check out equipment on Thursday, it is due back Tuesday.

*Some equipment may have extended checkout periods.  If you are in a class in which you have access to extended equipment checkout periods, the Instructional Support Technician will note the due dates on your Equipment Checkout Form.

Equipment Damage: As specified in the FTVM Equipment Checkout Agreement, students will be responsible for replacing and damaged or missing equipment.  The Lab Tech will provide the student a replacement cost and the student will submit the replacement fee to the department.

Upon Checkout, it's the student's responsibility to ensure that equipment he/she is checking our is functioning properly and the package is complete. Take the time to test all gear so that you can catch, and not later be held accountable, for damage or missing parts.

Late Fees:  If you fail to return your equipment on your scheduled return date, you will be assessed a $10 fee per each day that it is late.
