The Petition for the Associate Degree is for students applying for an associate degree. Fill out the top portion of the form and meet with a counselor to determine eligibility and to complete the bottom portion of the form. Submit the completed form to Admissions and Records.
The Petition for the Certificate is for students applying for a certificate. If you are completing CSU GE Breadth or IGETC as part of your transfer requirements, you are eligible to earn a certificate of achievement. Applying for the certificate will also show the full certification on your transcript.
Fill out the top portion of the form and meet with a counselor to confirm eligibility. If eligible, submit the completed form to Admissions and Records. The title of the certificate will be either "IGETC" or "CSU GE Breadth," depending on the plan you've been following.
The College Report/Transfer Report Request is for students who are applying to colleges that require either the College Report for the Common Application or the Transfer Report for the Coalition Application.
In order to get the College Report or Transfer Report completed, students must first fill out the College Report/Transfer Report Request Form and the top portion of the College Report or Transfer Report. It is students' responsibility to make sure that they provide accurate information for each college.
Students will then email the completed Request Form along with the completed College or Transfer Report to the Dean of Student Affairs, Tzoler Oukayan, at toukayan@glendale.edu.
GCC will send the report on behalf of the student to each college requested.
These forms are for students who are applying to colleges that require either the Mid-Term Report for the Common Application or the Curriculum Report for the Coalition Application.
Students fill out the top portion of the Mid-Term Report or Curriculum Report and present the form to instructors whose courses they are enrolled in at the time of application. After collecting information and signatures from faculty, students mail this form to the colleges requesting the form.
The Request to Count the Higher Grade allows students who received a "D," "F," or "NP" grade for a course, but repeated it at a later time at Glendale Community College or another college to request to have the previous grade value removed from their GPA.
Only the most recently earned grade and grade points will be used. Previous substandard grade(s) will be removed from the cumulative grade point average and cumulative total units. To ensure the integrity of the academic history, the substandard letter grade originally assigned for the course will remain visible on the transcript.
When a course was repeated at an institution other than Glendale Community College, students are required to submit a course description from the other college’s catalog and an official transcript showing successful completion of the course.
Note: Transfer institutions have their own policies when it comes to repeats; therefore, some institutions may include the "D" or "F" grade in the GPA calculation even if students repeated the course for a better grade.
The Petition for Academic Renewal allows students to petition to have "D" or "F" grades disregarded (excluded) from their GPA without having to repeat the course. Remember: it's always best to talk with a counselor to see if a course is worth repeating or not). Note that the substandard grade(s) will still remain on the transcript with a notation that the grade was excluded through academic renewal.
In order to qualify for academic, students must meet the following criteria:
- There must be a lapse of at least one semester (Fall or Spring) since completion of the most recent coursework to be disregarded
- Have completed 12 graded (A-F) semester units with at least a 2.0 GPA
- These 12 units must be consecutive and must have begun any time after the course work to be disregarded
Students can request academic renewal multiple times, for up to 30 units of substandard coursework.
Note: Transfer institutions have their own policies when it comes to academic renewal; therefore, some institutions may include the "D" or "F" grade in the GPA calculation even if students received academic renewal.
The Transcript Evaluation Request is for students who previously attended other colleges and wish to apply this credit toward a degree, certificate, or general education requirements.
Students should send their official transcripts from all colleges attended. Students must provide course descriptions (from the year the courses were taken) from all out-of-state and all private colleges within California. In addition, course outlines (syllabi) for all English, math, and speech courses from out-of-state and private colleges within California must be included. Note: Courses from colleges or universities in foreign countries cannot be used for IGETC or CSU GE Breadth.
Students should make an appointment with a counselor to fill out the bottom portion of the form. Counselors will then submit the completed form to Admissions and Records for evaluation.
Once you receive the completed evaluation through your Glendale College email account, make a follow-up appointment with your counselor to complete your Student Education Plan.
The Petition for AP Credit is for students who passed Advanced Placement (AP) exams with a score of 3 or higher, and wish to apply this credit toward a degree, certificate, or general education requirements.
Students should send their official exam score reports and meet with a counselor to complete this form. Submit the completed form to Admissions and Records.
The Petition for IB Credit is for students who passed International Baccalaureate (IB) Higher Level (HL) exams with a score of 4-5 or higher, and wish to apply this credit toward a degree, certificate, or general education requirements.
Students should send their official exam score reports and meet with a counselor to complete this form. Submit the completed form to Admissions and Records.
The Petition for CLEP Credit is for students who passed College Level Examination Program (CLEP) exams with a minimum score of 50 (for some exams, this minimum is higher), and wish to apply this credit toward a degree, certificate, or general education requirements.
Students should send their official exam score reports and meet with a counselor to complete this form. Submit the completed form to Admissions and Records.
The ADT Verification Form is for students who applied to the CSUs and are earning an Associate Degree for Transfer (ADT), but:
- Did not include the degree on their CSU application or
- Reported the incorrect degree
Students do not need to submit this form if they indicated on their CSU application that they are earning the ADT. GCC will provide this verification electronically.
Before completing this form, students must contact each CSU campus to which they applied and request to update their information. If permitted to do so, they then follow up with a counselor to confirm their eligibility and to obtain a signature on this paper verification form. Students then submit this verification to each CSU campus. Note: Some campuses may not accept this form.
